New Patient Registration

We welcome all new patients to Kindness Animal Hospital and thank you for entrusting your pet's healthcare to us.  To speed up and simplify the registration process we have given you three choices:

1. You can register online - fill out the information and click "Submit" at the bottom of the form.  When you come in for your appointment, the receptionists will already have your pet?s file made up and you will just need to review the information on the form and sign it.  If you submit the form on the same day as your appointment please call our office to let the receptionists know that they should look for the form.

2. You can click here to download and print the New Patient Information Form (if you do not already have AdobeReader® installed on your computer, Click Here to download.) and then print the form.  Just fill the form out ahead of time, fax it to us, drop it off before your appointment or just bring it in when you come in for your appointment.

3. Finally, you can still just fill out the form when you come in for your appointment.  We just ask that you come in 15 minutes early to allow us to input all the information and create the file for your pet.
